Request Forms

The best way for a student, staff, or faculty member to obtain services from Building Maintenance is to complete the Work Order Request form (link is below). The forms are immediately emailed to our routing personnel and forwarded on to our Maintenance Crew. The spaces in these forms can be filled out by students, staff and/or faculty to request minor maintenance repairs. The work requested will be performed during the regular maintenance schedule unless there is an immanent need or dangerous condition. Upon completion of the work, a card will be left on the door where the work was performed with any applicable notes from the maintenance staff. If the maintenance request is not completed, or if further questions arise, another request should be completed. Our maintenance personnel will make contact the following week.

The work requested will be done as soon as possible based on the following criteria:

  • Priority 1. Hazards to life, health, property, or programs.
  • Priority 2. Hazard may develop if it is not corrected at the next available opportunity.
  • Priority 3. Common repairs to be done at the next available date.

The Maintenance Crew will work closely with the Building Administration (academic centers) and Resident Directors (residence halls) to keep them informed of work outside the normal maintenance schedule being performed in their buildings. Resident advisors and students residing in the residence halls should direct questions regarding scheduled maintenance issues and/or service requests to their RD. If an RD has questions regarding the status of a request, the RD should contact Environmental Services at x8827. If the RD still feels that the issue is unresolved, he or she is encouraged to contact David Brown at x8888.

NOTE: Service requests filled out by faculty, residence hall staff or residents are considered to provide permission to enter residential spaces and/or offices to affect the requested repairs.
