The Center for Professional Development Forms

These forms are for the use of education administrators and teachers who are facilitating the instruction of NNU continuing education and professional development courses. Courses must be approved by the appropriate NNU Department Chair or Director before courses are advertised or student registrations can be accepted.  Please allow 2-3 weeks for these approvals. 

Please read the Course Criteria for Professional Development Credit before submitting your Course Request Form and Syllabus.  One graduate professional development credit requires 15 hours of direct teacher facilitated instruction and 30 hours of additional course work.
