Mission Statement

The primary mission of the Office of Health Services is to promote the optimum health of the student body through prevention, treatment, health promotion, and educational outreach. We strive to prepare students to be their own health advocates and informed consumers of appropriate health care services.

Services

Northwest Nazarene University Wellness Center provides confidential health services for all full-time students. Health Services provides the following:
    •Assessment and treatment of acute and chronic medical illnesses and injuries
    •Basic First Aid
    •Health Screenings
    •Annual & Sports Physicals
    •Lab services provided by Interpath Lab
    •Clinical support for routine treatments (per physician order)
    •Health promotion (including prevention, exercise, nutrition, stress management)
    •Referrals to community resources

Visits and Appointments

Most students are seen by the school nurse on a walk-in, first come, first served basis during clinic hours. Appointments may be scheduled by calling (208) 467-8466 or (208) 467-8673.

Confidentiality

Student Health Services maintains a policy of strict confidentiality for all services and medical records. Information will be released only with the written consent of the student or as required by law.

Health Records and Immunization Policy

Health Services is dedicated to a healthy campus environment; current health information and immunizations are a vital part of that goal.  A Health and Medical History Form and documentation of Immunization Records are required of all students.

DOMESTIC STUDENT IMMUNIZATIONS

There are 3 REQUIRED vaccinations that need to be included in immunization records:
1)      TDaP/Td (Tetanus/Pertussis) - within last 10 years
2)      Meningitis
3)      MMR (Measles/Mumps/Rubella) - Dose #1 & #2
         OR a blood titer to show immunity is required

Immunization Exemption Form

INTERNATIONAL STUDENT IMMUNIZATIONS

There are 5 REQUIRED vaccinations that need to be included in an International Student's immunization records:
1)      TDaP/Td (Tetanus/Pertussis) - within last 10 years
2)      MMR (Measles/Mumps/Rubella) - Dose #1 & #2
         OR a blood titer to show immunity
3)      Polio - 4 doses in childhood or 3 adult doses 
4)      Tuberculosis Screening
5)      Meningitis
